1、Sichuan Journal of Anatomy2023,31(2):9-11著四川解部学杂志复发性流产患者血浆低氧诱导因子-1、淀粉样蛋白A、蛋白S活性水平变化及预测价值海河许昌市中心医院检验科,许昌46 10 0 1【摘要】目的:探讨复发性流产(RSA)患者血浆低氧诱导因子-1(H IF-l)、淀粉样蛋白A(SA A)、蛋白S活性水平变化及预测价值。方法:选择2 0 2 0 年6 月至2 0 2 2 年6 月在本院进行产前检查的9 3例RSA患者(自然流产次数2 次)为RSA组,选择同期既往无自然流产史健康早孕产检孕妇9 3例为对照组。比较两组血浆HIF-1、SA A、蛋白S活性水平,
2、分析血浆IF-1、SA A、蛋白S活性水平临床意义,以及三者联合诊断RSA的临床价值。结果:与对照组相比,RSA组血浆HIF-1、SA A 水平较高,蛋白S活性水平较低,差异有统计学意义(P0.05)。受试者工作特征(ROC)曲线分析显示,血浆HIF-1、SA A、蛋白S活性水平联合预测RSA的ROC曲线下面积(AUC)最大(AUC=0.893),且敏感度(8 4.9 5%)、特异度(9 4.6 2%)最高,差异有统计学意义(P0.05)。结论:血浆HIF-1、SA A 水平在RSA患者中呈高表达,蛋白S活性呈低表达,联合应用时可有效预测RSA发生,为临床早期预防提供依据。【关键词】复发性流产

6、re selected as the controlgroup.The plasma HIF-1,SAA and protein S activity levels were compared between the two groups,and predictive valuefor the development of RSA were analyzed.Results:Compared with the control group,plasma HIF-1 and SAA levelswere higher and protein S activity levels were lower

9、S我国妊娠妇女中复发性流产(recurrentmiscarriage,RSA)的发病率为0.5%10.0%,呈逐年升高趋势,且RSA患者再次妊娠时的自然流产率可达50%7 0%。近年研究指出,机体抗凝和纤溶系统失衡引起血液高凝状况或者微血栓状况,可导致子宫-胎盘血的灌注量下降,进而使胚胎组织出现缺血缺氧,最终引起RSA等不良结局。蛋白S为临床一种反映凝血功能的指标,可能在RSA发生中发挥一定作用2 。淀粉样蛋白A(a my l o i d A,SAA)在调节滋养层细胞迁徒、加快胎盘形成与维持胎盘内稳态具有重要作用,胎盘微环境中,SAA含量过高,可经调节金属蛋白酶活性、滋养层侵袭有海河,主管检验
