1、干细胞的突变与肿瘤干细胞是一类具有自我更新和增殖分化能力的细胞,肿瘤干细胞是存在于肿瘤组织中的一小部分具有干细胞性质的细胞群体,能够驱使肿瘤的形成。不同的是,干细胞的增殖具有相对稳定性,其数目保持相对恒定,而肿瘤细胞虽可以无限增殖,但却失去了自稳定性的特点。其机制可能是由于干细胞基因突变及突变累积、非整倍体扩增、不对称分裂、端粒酶作用以及信号转导途径和微环境异常导致干细胞增殖分化机制失调1。早期的研究表明,单一细胞获得47次基因突变将发生恶性转化2。组织更新快的上皮组织、造血系统是肿瘤高发部位,组织自我更新越快,复制、转录过程中基因发生突变的概率越高。由于干细胞具有无限增生能力,在体内可长期存
2、在,这使基因突变更容易在干细胞中发生和积累。已有报道指出某些结肠癌和白血病产生于积累了多次突变的干细胞3, 4。结直肠上皮所有的细胞来源于隐窝底部46个干细胞,干细胞不断向表面增生,干细胞增生形成分化细胞的数量和分化细胞死亡或脱落的数量维持平衡。用放射线诱导突变人类肠隐窝细胞发生表型变化大约需1年时间,分化细胞仅有2天的寿命,而1年正好是单一突变的干细胞增生形成肿瘤的时间。越来越多的证据表明,肿瘤产生于积累了多次突变的组织特异性干细胞或骨髓衍生的间充质干细胞(MSC)的恶性转化5-7。如果人体内存在积累了多次突变或恶性转化的干细胞8,它们是可能通过遗传传递给后代的,这也许是存在高癌发病家族的一
3、种可能的原因和解释9。最近,Nat Cell Biol发表的1篇文章阐述了老化过程中积累的基因组损伤对干细胞功能的影响10。干细胞和体细胞中年龄依赖的DNA损伤积累可能是老化的干细胞功能障碍的原因。干细胞具有长期的自我更新能力,然而这种能力同时伴随着固有的风险:由于干细胞是生命体中最长寿的细胞,那么获得基因损伤的风险因而也增高。DNA损伤通过突变或染色体重排改变基因功能,如调控干细胞分化和自我更新的基因失调,肿瘤抑制子失活或癌基因活化2, 11。因而,干细胞癌变是极其危险的。一系列研究在人造血干细胞(HSC)中鉴别到了一个年龄依赖的突变累积12, 13,果不其然,这些突变频繁出现在白血病中,且
4、早于疾病症状的出现13, 14。在造血系统中,老化的特征是淋巴组织生成减少,骨髓细胞生成增加,而HSC组分的克隆漂移可能与这些变化有关15, 16。HSC包含不同的亚群,包括偏淋巴的HSC和偏骨髓的HSC。在老化过程中,偏淋巴的HSC减少,而偏骨髓的HSC保持不变,尽管后者在单细胞水平表现出减弱的功能16。HSC无性系组成的漂移被认为是导致老化过程中免疫功能下降和髓性白血病风险增加的原因17。最近,Lundberg等在J Exp Med上发表的一篇文章表明携带JAK2-V617F突变的单个HSC能够启动骨髓增生性肿瘤(MPN)的发生18。目前,干细胞转化为肿瘤干细胞的具体机制以及各种肿瘤组织中
